Scheduling:

Online scheduling is required. If you cannot find an appointment time that works for you, then please reach out and I will be happy to help try to accommodate your needs.

Cancellation and No Show Policy:

It is our policy that a credit card be placed on file at the time of booking. This card will only be charged if the appointment is cancelled/rescheduled LESS than 24 hours before your appointment or you do not show up for your appointment. If you are more than 15 minutes late for your appointment it may be necessary to reschedule. This will be deemed a late cancellation and is subject to a $50 fee. This fee is non-refundable.

Pre-Appointment Paperwork:

When your appointment is booked, shortly after you will receive an email with a Consent form and possible Medical History form. Please fill these forms out in advance to ensure that we are creating a safe treatment plan for you.
